Responsible for the overall management of the home health and hospice clinical services including the day to day operations, planning and development, oversight of client care and staff, as well as staff education and mentoring.


  • Oversee skilled nursing care, hospice, therapy, MSW, nutritionists and agency services, through consultation, case conferences, clinical in-services, Interdisciplinary Group (IDG) and other meetings
  • Participate in and facilitate performance improvement efforts in collaboration with other nursing management staff
  • Complete competency and job performance reviews of all clinical staff through use of clinical documentation, QAPI activities and field visits at least annually as needed
  • Facilitate follow-up and resolution to issues that arise
  • Respond to and follow up on client and staff complaints/concerns and incidents
  • Perform coaching/training visits with staff
  • Ensure clinical practice and documentation by clinical employees complies with state and federal regulations, accreditation standards and agency policies and procedures

  • Provide on-going education in response to identified performance improvement opportunities
  • Share in 24-hour supervisory extended hour call for weekends and after hours
  • Ensure optimal cost effectiveness in staff utilization and participate in decision making regarding organization of service delivery and human resources hiring
  • Participate in individually assigned committees
  • Review and ensure accurate, complete comprehensive assessment and completion of client/family record at time of admission, change of condition, resumption of care, in-patient stay, re-certification and discharge
  • Respond to phone calls from field staff, payers, physicians, clients and families
  • Participate in clinical policy and procedure development to meet on-going agency needs
  • Collaborate with administrative support team and business office to maximize efficiencies of processes related to the provision, documentation and reporting requirements of services
  • Chair home health and hospice nurse meetings and case conferences
  • Screen referrals for appropriateness of hospice services, method of payment, attending and managing physician and safety

  • Foster collaboration between all disciplines through interdisciplinary group meetings (IDG) and on a day to day basis to ensure optimal appropriate care for hospice patients and their families
  • Fulfill all functions of registered nurse for home health and hospice
  • Facilitate Hospice Nurse on-call coverage after hours and on weekends
  • Responsible for Quality Reporting audits, Performance Improvement plans, education, tracking and reporting
  • Perform and assist in QAPI activities including auditing and participation in performance improvement
  • Review physician orders and ensure follow through

  • Attend association meetings
  • Provide back-up visits when necessary



  • Successful completion of an accredited Registered Nursing Program; current license through the State of Michigan State Board of Nursing as a Registered Nurse
  • Two (2) years of nursing experience with current home health and/or hospice
