  • Overview

    Provide skilled and non-skilled nursing services to home care and hospice patients in accordance with nursing standards and regulatory rules.  These functions include assessment, procedures, teaching, case management, documentation and supervision of non-skilled personnel.  Provide evening, on-call and weekend staffing as required by Home Care.  Act as an advocate for the patient and family to ensure comfort, relief and hope.


    • Seek and obtain sufficient information necessary to function effectively in the healthcare setting where assigned

    • Provide professional nursing care by utilizing all elements of nursing process

    • Delegate to team members focusing on areas of expertise and professional strengths

    • Respond timely to phone calls from field staff, payers, physicians, clients and families

    • Revise Plan Of Care as client condition changes

    • Maintain communication with other disciplines to coordinate the POC

    • Assure supervision of the home health aide is done every 2 (two) weeks

    • Participate in QA activities as assigned

    • Assess and evaluate client’s status by:

    • Providing an accurate, complete comprehensive assessment at admission, re-certification, change of condition, return from hospital inpatient stay and discharge

    • Developing, reviewing and implementing the POC

    • Regularly re-evaluating client needs

    • Provide instruction as appropriate

    • Administer medications and treatments as prescribed by the physician

    • Initiate and apply appropriate preventative, therapeutic and rehabilitative nursing procedures and techniques

    • Maintain communication regarding client care with the physician, supervisory personnel, and other disciplines

    • Complete, maintain and submit accurate and relevant documentation of client’s condition and care provided according to procedures

    • Use supplies and equipment effectively and efficiently

    • Observe the client for changes in condition, discuss changes with the team leader, client or family/caregiver, and promptly report changes to the appropriate physician

    • Assist the client with meeting the activities of daily living and facilitate the client’s efforts toward self-sufficiency and independence

    • Teach the client and family/caregiver self-care techniques as appropriate to meet their physical, emotional, spiritual, social, and safety needs; provide medication, diet and other instructions as ordered by the physician and recognizes and utilizes opportunities for health counseling with clients and families/caregivers

    • Maintain client confidentiality according to agency and HIPAA policy

    • Provide and maintain a safe environment for the client

    • Assist the client’s family/caregiver and other health care personnel in providing continuity of care

    • Supervise the LPN and ancillary personnel and delegate responsibilities when required

    • Attend and contribute to staff meetings and education programs as required

    RN Case Manager

    • Maintain caseload based on work status

    • Prepare written communication and attend Case Conference –QOW

    • Facilitate follow-up and resolution of “visit book” problems for case load

    • Review HHRG with Supervisor/Oasis Analyst prior to 485 mailing

    • Review all Admits/Recerts/Follow Up with team to set up plan

    • Manages own schedule and patient assignments to assure continuity to meet client’s needs

    • Manage cases in assigned geographical area utilizing team members, aides, and other disciplines

    • Assign visits to team nurses if unable to see own client

    • Oversee client schedule to assure all visits are assigned

    • Audit charts on a regular basis for completeness, accuracy and compliance

    • Follow up on hospitalized clients at least weekly by phone to determine if client still remains admitted to facility



    • Assist with organization and time management

    • Perform clinical skills assessment with documentation of competency

    • Ensure opportunities of learning are utilized

    • Assist with the development of communication and problem solving skills

    • Provide constructive and timely feedback regarding progress to both the mentee and the supervisor on a weekly basis

    • Maintain a formal and supportive relationship for 3-6 months, determined by the level of competency and comfort as agreed upon by both the mentor and mentee and the supervisor, then monthly thereafter for the next six months



    • Successful completion of an accredited Registered Nursing program; current license through the State of Michigan Board of Nursing as a Registered Nurse

    • Two (2) years of nursing experience with current home health and/or hospice,  medical, surgical, or critical care experience preferred
