  • Overview

    Responsible for the overall short term and long term admission process in order to ensure budgeted census targets and conversion ratios are met/exceeded.  Responsible for determining the ability for the facility to meet the clinical and physical needs of the resident referral.  Facilitates the overall admission process from inquiry to admission as well as acts as the resources for hospitals, physicians, family and patients.  Communicates all admissions with key nursing and administrative personnel.


    • Responsible for ensuring monthly and annual census goals are met/exceeded
    • Authority to accept/deny patient referrals
    • Review all referrals and respond timely to referral source
    • Meets monthly with Community Liaison Supervisor in order to review Revenue Tracking Log
    • Reviews conversion ratios and referral source trends
    • Interacts with the hospital discharge planner meetings
    • Provides representation for bundled payment and population health meetings held by key hospitals
    • Attend hospital meetings as directed
    • Travels to hospital in order to conduct assessment (if needed)
    • Determine organization’s clinical and financial ability to meet the needs of potential patients/residents including high cost medication, treatment needs and equipment needs
    • Maintain up to date knowledge as it relates to current regulations governing all payers
    • Maintain accurate referral information in appropriate database, i.e., IRM and AllScripts
    • Assist with completion of documents based on current policy and procedure
    • Complete insurance verification and pre-certifications
    • Develop good working relationships hospital discharge planners and other referral sources
    • Maintain occupancy and payer mix based on current budget
    • Obtain required insurance and clinical information and submit to appropriate department(s)


    • Successful completion of an accredited Licensed Practical Nursing or Registered Nursing program; currently licensed by the State of Michigan Board of Nursing as a Licensed Practical Nurse or Registered Nurse preferred
    • Minimum of three years of experience in health care sales / marketing / insurance or similar field preferred.
