  • Overview

    Support the stability of foster care and Enhanced Foster Care program placements by proactively developing, implementing and monitoring behavioral plans and logs with children in foster care and their foster parents.   Provide assessment and appropriate intervention as necessary.


    • Demonstration of the following in the position:
      • Sensitivity to and awareness of cultural, ethnic and socioeconomic difference
      • Interpersonal skills necessary to interact with and engage a diverse group of high risk families
      • Knowledge of community services for at-risk families
      • Ability to assess individual and family strengths, limitations and needs
      • Non-judgmental, strength based, positive attitude towards families in crisis
      • Ability to synthesize information and write thorough reports
      • Ability to work independently and as an effective team member
    • Develop, coordinate and implement concise, accurate plans, including behavioral plans, with foster youth and families that provide support and stability of placement for identified youth.
    • Review behavioral log(s) with foster care parents regularly, to ensure implementation and effectiveness of behavior plan
    • Ensure barriers are addressed that may impede parents and foster children’s ability to meet all goals and objectives on the treatment plan.
    • Respond to after-hours crisis calls and provide telephone consultation, referral, crisis intervention services and make recommendations
    • Assess level of need/urgency and respond accordingly, to families experiencing a crisis that may disrupt foster placement
    • Provide short-term intervention on an individual, family, couple or group level to stabilize living situation, where possible.
    • Coordinate the delivery of services with other service providers both internally and externally to the agency, including area professionals and provide testimony in court where required.
    • Facilitate psychoeducational groups, as needed.
    • When the agency receives a 30 day notice to move a child from their current residential program and/or it is determined that a different residential placement is needed, work to identify an alternate placement appropriate to the child’s needs and facilitate transition. When a foster child’s treatment team has determined that the child can no longer safely remain in a foster home or community placement, work to identify an appropriate residential placement to best meet the foster child’s needs and facilitate the transition.
    • Complete data entry into all data management systems as required, as well as keep paper files accurate, complete, and in compliance with policy, rules, and state law.



    • Bachelor’s degree in a human services field
    • Two years of experience working with children and families preferred
