  • Overview

    Provide personal assistance, ADLs, homemaking services and promotes patient comfort and safety.  Responsible for observing and reporting/documenting the observations and services provided.  The Certified Nursing Assistant (C.N.A.) is assigned in a manner that promotes quality, continuity and safety of patient care.


    • Provides services ordered by the physician and included in the Plan of Care (POC)
    • Follow plan of care to meet patient’s needs; Contacts the Registered Nurse or other appropriate skilled professional if unable to perform a requested task, or to report any changes in patient condition
    • Honors patient rights
    • Meet safety needs of patients and family and use equipment safely and properly
    • Give personal care including Activities of Daily Living (ADLs), baths, showers, backrubs, oral hygiene, nail care with emery boards, nail clippers and orange sticks, shampoo patient’s hair, and change bed linens as specified on the plan of care, respecting patient’s privacy and property
    • Assist in feeding the patient as necessary
    • Assist in dressing and undressing the patient
    • Plan and prepare nutritious meals
    • Take and record oral, auxiliary temperature, pulse, respiration and blood pressure as directed and competency tested
    • Recognize and report changes in skin condition
    • Provide proper care and observation of patient’s skin to prevent breakdown
    • Assist in ambulation and exercise as instructed by the primary nurse or therapist
    • Offer and assist with bedpans, urinals, toileting and incontinence care and enemas with prepared enema (Fleets type)
    • Perform range of motion and other simple procedures as directed within scope of practice and for which competency has been verified
    • Assist the patient and family in the self-administration of medication as directed
    • Maintain a clean, safe and healthy patient environment
    • Do patient laundry as directed
    • Adhere to organizational documentation, care and practice policies and procedures and protects patient’s privacy. Observes, reports and documents patient status and the care or service furnished
    • Attend the required educational presentations as required by agency
    • Follow standards of universal precautions and infection control according to policies and procedures
    • Conduct self in a professional manner and maintain the ethical standards of the organization
    • Perform additional tasks as directed by RN supervisor and for which competency has been verified
    • Maintain a communication method by which agency can contact for scheduling i.e. telephone and pager; responds to voice mail messages in a timely manner
    • Apply appropriate agency non-medication protocols for pain and symptom management as directed and competency tested
    • Observe client and promptly report changes to the supervisor or Skilled Nurse
    • Apply non-prescription ointments
    • Perform range of motion exercise per Plan of Care (POC)
    • Demonstrate competency in tasks required by the Conditions of Participation
    • Follows all precautions included in the Plan of Care (POC)
    • Provide catheter and colostomy care, perform simple dressing or other special tasks as directed after competency has been verified
    • May assist client with set up and cleaning of equipment such as oxygen and nebulizers
    • Change linens and perform light housekeeping
    • Prepare light meal or snack, offer fluids if directed; assist in feeding client as directed
    • Demonstrates ability to read, write and verbally report clinical information to patients, representatives, caregivers and HHA staff
    • Recognizes emergencies and knowledge of instituting emergency procedures and their knowledge


    • High school diploma or equivalent preferred
    • Has successfully completed a State established or other training program and a competency evaluation program for Certified Nursing Assistants
    • Must be re-tested or re-trained if there has been a 24 month lapse in furnishing services for compensation
    • Is currently listed in good standing on the State of Michigan nurse aide registry
    • Completes 12 hours of in-service training during each 12 month period
    • Minimum of 1 year experience as C.N.A. preferred
