Registered Nurse (RN) - Per Diem - Home Health & Hospice - Per Diem

Provide skilled and non-skilled nursing services to hospice clients in accordance with nursing standards, and regulatory rules.  These functions include assessment, procedures, teaching, case management, documentation and supervision of non-skilled personnel.   Provide evening, on-call and weekend staffing as required by Home Care.  Act as an advocate for the client and family to ensure comfort, relief and hope.

• Seek and obtain sufficient information necessary to function effectively in all hospice settings where assigned
• Provide professional nursing care and comfort by utilizing all elements of nursing process
• Assess and evaluate client and family status by:
• Providing the initial client assessment
• Reviewing and implementing the Plan of Care to ensure pain and symptom relief
• Regularly re-evaluating client/family needs to ensure comfort, relief and hope
• Revising the services delivery plan of care as necessary
• Participating in the interdisciplinary group meetings
• Administer medications and treatments as prescribed by the physician
• Initiate and apply appropriate preventative, therapeutic and rehabilitative nursing procedures and techniques; maintain communication regarding client care with the physician, supervisory personnel, and other disciplines
• Complete, maintain and submit accurate and relevant clinical notes regarding client’s condition and care given according to procedure
• Use supplies and equipment effectively and efficiently
• Observe the client for changes in condition; discuss changes with the client and family/caregiver; report changes as promptly as necessary to attending physician and other team members
• Assist the client with meeting the activities of daily living and facilitate the client’s desire for self-sufficiency, comfort and independence
• Teach the client and family/caregiver self-care techniques as appropriate to meet their physical, emotional, spiritual, social, and safety needs 
• Provide medication, diet and other instructions as ordered by the physician; recognize and utilize opportunities for health counseling with clients and families/caregivers
• Provide and maintain a safe environment for the client
• Assist the client’s and family/caregiver and other health care personnel in providing continuity of care
• Supervise ancillary personnel and delegate responsibilities when required
• Provide nursing care to the clients as requested by the client and family, when required, as prescribed by the physician and in compliance with applicable laws and regulations and the policies of the organization

Mentor

• Assist with organization and time management
• Perform clinical skills assessment with documentation of competency
• Ensure opportunities of learning are utilized
• Assist with the development of communication and problem solving skills
• Provide constructive and timely feedback regarding progress to both the mentee and the supervisor on a weekly basis
• Maintain a formal and supportive relationship for 3-6 months, determined by the level of competency and comfort as agreed upon by both the mentor and mentee and the supervisor, then monthly thereafter for the next six months

• Successful completion of an accredited Registered Nursing Program; current license through the State of Michigan State Board of Nursing as a Registered Nurse

• Ability to bend, stoop, sit, stand, reach, and lift items weighing 100 pounds or less
