The Foster Care Family Finder plays a crucial role on the foster care team searching for, documenting, and engaging the families and support systems of the children in our care.
This position is expected to function effectively with moderate supervision while following the guidelines given on procedures, along with agency, federal, and state regulatory requirements.

Assess youth and families in order to develop, coordinate and implement individualized treatment plans with youth and their families to resolve abuse and neglect difficulties and to achieve permanency for youth; Coordinate youth’s placement within foster care, school and community, DHS and family court. Manage a caseload of approximately 15 children; Testify in court; Provide documentation for all activities completed.
Assist residents with personal care, mobility to and from the dining room and life enrichment activities. Encourage residents and support their chosen life style, ensuring a safe and enjoyable living environment. Medication Administration Persons (MAPs) also perform overall medication administration, measurement of related vital signs and blood sugars, simple treatments and documentation in addition to caregiving duties.
Provide support, clinical supervision, training and development, and oversight to a HOMEBUILDERS® team of workers. Supervisor activities include: Organize workflow and priorities; Assign work among employees; Maintain quality/quantity standards regarding program performance and outcomes; interpret operating policies and procedures; Participate in hiring process; Train employees; Evaluate employee performance; Twenty-four-hour (24) availability to accept referrals or assist with staff and client emergencies. Serve as primary liaison between HOMEBUILDERS® team and the agency’s management team. Serve as the primary contact with the HOMEBUILDERS® Consultant regarding implementation of the model and program fidelity. Be available to respond to client and worker crises as they occur. This position reports to the Program Manager.

Provide coordination, management and direct clinical services for substance use disorder recovery program serving older adults in post-acute residential setting (skilled nursing facility). Position will provide assessment, service planning, intervention, interdisciplinary team consultation, assistance with integrating social services with other disciplines for total resident care, supervision and management of peer coaching staff, aftercare and referral support, for patients with Substance Use Disorders (SUDs) and their families; Position also facilitates recovery oriented activities on site which emphasize overall wellness.

Provide service excellence to the residents and staff by maintaining the Ministry Site building and grounds and completion of Job Orders for maintenance of the buildings and outside grounds.

Serves as a liaison between current and prospective foster parents and their licensing agencies. Works to empower, educate and advocate for families who wish to become or continue their role as foster parents. Plans, coordinates, and attends foster family recruitment activates according to contracted requirements. Plans and facilitates foster parent retention activities according to contracted requirements.
Assist foster care staff in meeting the needs of clients by transporting children and supervising interactions between children and their families; monitor, supervise, and coach parents during parenting times; assist foster care staff in case related activities.
Responsible for developing positive relationships with youth that encourage achievement of treatment goals. Maintain the treatment milieu by providing for the youth’s basic needs, being effective in counseling and managing the youth’s behavior in accordance with the youth’s treatment plan, and working well in teamwork with youths’ families, senior youth specialists and other staff members.
