Assess youth and families in order to develop, coordinate and implement individualized treatment plans with youth and their families to resolve abuse and neglect difficulties and to achieve permanency for youth; Coordinate youth’s placement within foster care, school and community, DHS and family court.  Manage a caseload of approximately 15 children; Testify in court; Provide documentation for all activities completed.

Medication Administration Persons (MAPs) also perform overall medication administration, measurement of related vital signs and blood sugars, simple treatments and documentation in addition to caregiving duties.

Responsible for developing positive relationships with youth that encourage achievement of treatment goals.  Maintain the treatment milieu by providing for the youth’s basic needs, being effective in counseling and managing the youth’s behavior in accordance with the youth’s treatment plan, and working well in teamwork with youths’ families, senior youth specialists and other staff members.

In this work from home position, you are responsible for scheduling appointments to help secure new customers across several states. This involves taking company provided leads and sending compelling emails, making calls and scheduling webinars.   This position is part time (20 hours per week) Monday through Friday with 4 hour shifts between the hours of 9:00am – 6:00pm. Provide skilled and non-skilled nursing services to hospice clients in accordance with nursing standards, and regulatory rules.  These functions include assessment, procedures, teaching, case management, documentation and supervision of non-skilled personnel.   Provide evening, on-call and weekend staffing as required by Home Care.  Act as an advocate for the client and family to ensure comfort, relief and hope.

Provide skilled and non-skilled nursing services to home health patients in accordance with nursing standards and regulatory rules.  These functions include assessment, procedures, teaching, case management, documentation and supervision of non-skilled personnel.  Provide evening, on-call and weekend staffing as required by Home Care.  Act as an advocate for the patient and family to ensure comfort, relief and hope. This position is considered part of Skilled Professional Services and is subject to the Skilled Professional Services policy.
