We are looking for a motivated individual to work with families who are providing foster care placement for children. Complete home studies to determine families’ suitability to meet the needs of children in out-of-home care. Conduct rigorous background checks, home inspections and psycho-social assessments; Orient and train prospective foster families to the application, home study and placement process. Responsibilities could include making placement matches and coordinating the placement process. Monitor up to 30 licensed foster homes or enrolled families; Complete investigations to ensure foster families’ compliance with licensing rules, agency and state policies, and state law.

Carry a caseload of two (2) families. Responsible for: regular visits with the family in their home or community for a period of 28 days, and not more than 42 days; performing required assessments; writing and submitting service plans, safety plans and progress reports; providing concrete assistance and referrals based on the individual needs of each family. Work a flexible schedule (including non-traditional hours) based on families’ needs.

Responsible for developing supportive relationships with the families that live at the transitional home. Ensure families are following house roles, assist families in meeting goals of achieving self-sufficiency and independence. Ensure home is safe, secure, and well-maintained.

The Family Assistance Worker will assist families with issues such as parenting, child care, household management, nutrition and budgeting. In addition, the worker will assist supervision by transporting children and supervising interactions between children and their families; monitor, supervise, and coach parents during parenting times. Testifying at court regarding supervised visitation may be necessary.Insert any required credentials

Provide moderately-intensive, solution-focused, home-based family intervention, life skills education, and supportive services in order to prevent crisis situations that could threaten family stability and lead to removal. Carries a caseload of up to (8) families. Responsible for: regular visits with the family in their home or community for a period of 90 days, and not more than 180 days; performing required assessments; writing and submitting service plans, safety plans, and progress reports; providing concrete assistance and referrals based on the individual needs of each family. Works a flexible schedule (including non-traditional hours) based on families’ needs.

Assess youth and families in order to develop, coordinate and implement individualized treatment plans with youth and their families to resolve abuse and neglect difficulties and to achieve permanency for youth; Coordinate youth’s placement within foster care, school and community, DHS and family court. Manage a caseload of approximately 15 children; Testify in court; Provide documentation for all activities completed.

Assist foster care staff in meeting the needs of clients by transporting children and supervising interactions between children and their families; monitor, supervise, and coach parents during parenting times; assist foster care staff in case related activities.

Responsible for developing and implementing therapeutic treatment plans with families that will assist them in resolving the issues that led to a referral to the child welfare system focusing primarily on substance use and co-occurring disorders. Use counseling techniques, community resources, concrete services, and family empowerment to assist the family in goal achievement. Communicate effectively with the local DHHS, courts, medical and education systems to advocate for the family. Responsible for regular visits with the family in their home, administer required assessments and survey instruments; writing and submitting treatment plans, service plans, safety plans and court reports; assist in the arrangement of aftercare services based on families’ needs for a caseload of up to 8 families. Works a flexible schedule (including non-traditional hours) based on families’ needs.

Responsible for developing positive relationships with youth that encourage achievement of treatment goals. Maintain the treatment milieu by providing for the youth’s basic needs, being effective in counseling and managing the youth’s behavior in accordance with the youth’s treatment plan, and working well in teamwork with youths’ families, senior youth specialists and other staff members.
