Assess youth and families in order to develop, coordinate and implement individualized treatment plans with youth and their families to resolve abuse and neglect difficulties and to achieve permanency for youth; Coordinate youth’s placement within foster care, school and community, West Michigan Partnership for Children (WMPC), DHHS and family court. Manage a caseload of approximately 13 children; Testify in court; Provide documentation for all activities completed.
Complete home studies to determine families’ suitability to meet the needs of children in out-of-home care. Conduct rigorous background checks, home inspections and psycho-social assessments; Orient and train prospective foster families to the application, home study and placement process. Responsibilities could include making placement matches and coordinating the placement process. Monitor up to 30 licensed foster homes or enrolled families; Complete investigations to ensure foster families’ compliance with licensing rules, agency and state policies, and state law.
